Chronic Care Pharmacist

The core of our approach is understanding individual needs of chronically ill patients and their caregivers and building treatment plans and regimens in collaboration with their Primacy Care Providers. We start with a Comprehensive Medication review and build a plan for optimizing their current treatment with medications, identifying nutrient depletions and suggesting appropriate supplements to their PCP's.  Our service involves the preparation of a Medication Action Plan in a simple format that is easy to follow. 

  Of course, all of this is carried out via a series of virtual but face-to-face interactions. 

 I am a Savvy pharmacy entrepreneur and a practicing Pharmacist for nearly two decades and serve Business Customers interested in practice transformation at their retail community pharmacies or physician practice. My focus lies in implementation of Clinical services including immunization services and point of care testing.

I also accept patients and caregivers as clients, who require concierge pharmacist services including Medication Therapy Management ( MTM ), Diabetes Education and management and other pharmacist based services.

With my direct exposure and involvement for nearly a decade in planning and implementing Customer Care & Clinical Programs, Medicare STAR ratings, Quality Payment Program for Physicians, Pharmacy Workflow, Pharmacy ( HR ) Human Resource, Pharmacy Insurance internal audits, I am able to bring a quality focussed thinking and implementation for my business clients.
